89R3278 EAS-D By: Oliverson H.B. No. 1134 A BILL TO BE ENTITLED AN ACT relating to the prohibited maintenance of certain vaccine exemption records by the Department of State Health Services. BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS: SECTION 1. Section 161.0041(e), Health and Safety Code, is amended to read as follows: (e) The department shall maintain a record of the total number of affidavit forms sent out each year and shall report that information to the legislature each year.
