The Independence at Home Act of 2019 establishes a permanent Medicare program designed to provide comprehensive, coordinated, and continuous home-based primary care to beneficiaries. This program, called the Independence at Home Medical Practice Program, aims to reduce hospitalizations, readmissions, and emergency room visits while improving health outcomes and efficiency. Participating medical practices receive incentive payments if they spend less than an estimated target, and the program extends and expands an existing demonstration program, removing previous participant limits.
